Referral Forms
Referral forms are for providers referring clients/patients
on behalf of the patient.
Referrals from a doctor are not needed. If you are contacting us to schedule an appointment for yourself or a loved one, referral forms are not required. Just call or email the office and we will be happy to assist you.
Referrals may be faxed to 704-869-2047 or emailed to pattpsych@gmail.com.
