*Counselor availability varies. Please contact our office to see what available
appointments we have that will work with your schedule. We do have therapists
who provide some evening & weekend hours.
FEES (effective 10/13/2022):
New patient appointment (individual): $165.00
Follow-up appointments (individual): $125.00
Additional time can be requested for $31.25 per 15 minutes
New patient medication management: $200.00
Follow-up medication management: $135.00
Diagnostic Assessment/Evaluation/Testing Only: $250
Emotional Support Animal Assessment & Documentation = Fees vary (see packages here)
Additional Fees:
Late reschedule fee (less than 24 hour notice): $25
Late cancellation fee (less than 24 hour notice): $50 for the first occurrence, $100 for each concurrent occurrence
No show fee: $50 for the first occurrence, $100 for each concurrent occurrence
Records request (provided electronically): $15
Records request (provided in paper format, fees per page as dictated in GS90-411):
First 25 pages = $0.75/pg Pages 26-100 = $0.50/pg Pages 101+ = $0.25/pg
Completion of paperwork/report writing outside of session: $75.00 per hour
